As a social services worker at a long-term care facility, your primary responsibility is to support the emotional, social, and psychological well-being of residents and their families. NOW HIRING: SOCIAL SERVICES WORKER Your duties may include: Conducting initial assessments of residents to evaluate their social and emotional needs.

Developing individualized care plans in collaboration with residents, families, and other members of the healthcare team.

Providing counseling and support to residents and their families, particularly during periods of transition or adjustment.

Advocating for residents' rights and preferences within the long-term care setting.

Assisting residents in accessing community resources and services, such as financial assistance, legal aid, or recreational activities.

Facilitating support groups or educational sessions for residents and families on topics such as coping with illness, grief and loss, or caregiver stress.

Collaborating with interdisciplinary team members, including nurses, doctors, therapists, and administrators, to ensure holistic care for residents.

Documenting assessments, interventions, and progress notes in residents' medical records in compliance with regulatory standards.

Participating in case conferences or care planning meetings to discuss residents' progress and adjust care plans as needed.

Providing crisis intervention and support during emergencies or difficult situations involving residents or their families.
