JOB SUMMARY:
The Social Services Director coordinates the identification of medically related social and emotional needs of each resident, works to arrange for services to meet these needs, either by qualified staff of the facility or by referral to appropriate community agencies. Assists residents to adjust to illness, life changes and/or placement in the facility. The Social Services Director will be assisted by qualified staff in various aspects of direct social services. These staff members will be supervised and trained by the Social Service Director.
RESPONSIBILITIES AND DUTIES:
Obtain pertinent social history from residents and /or family members, including such information as background information on the family and home situation of each resident, and an assessment of the resident’s psychosocial needs and strengths.
Informs resident and/or family members about available services, facility programs and community-based programs. This education could include topics such as Medicare and Medicaid benefits, and Resident Rights.
Assists residents in obtaining pastoral care or spiritual support when such care is desired.
Assists residents and families as appropriate in resolving concerns, grievances or problems. Serves as a resident advocate.
Promotes appropriate family involvement with residents.
Participates with the care plan team in the development, implementation, and evaluation of the resident care plan by contributing relevant information.
Develops, implements and provides interventions on the social service aspects of resident care plans.
Accepts referrals for social service involvement from residents, staff, families, physicians and other appropriate referral sources.
Provides supportive visits to the residents and performs brief services such as communicating with family and friends, assisting to secure clothing, and enlisting the services of community resources as needed. Helps to facilitate the resident’s adjustment to illness, life changes, and nursing home placement.
Coordinates discharge planning with resident, family, staff, and outside agencies, as directed by the physician.
Informs Administrator of any reported, suspected, or witnessed mistreatment of a resident.
Maintains confidentiality in the handling of resident information and referrals in accordance with HIPAA standards.
Educates residents and family members on advance healthcare directives and documents advance directive status in the chart.
Attends appropriate in-services and continuing education.
Encourages residents to participate in individual and group social service programs and to utilize these resources to their best advantage.
Makes guardianship referrals as appropriate.
Interprets facility policy to residents and families.
Participates in quality improvement meetings and maintains professional standards of social service practice.
Provides training, education, and supervision of the Social Service Department to help assure that resident services and documentation are completed in accordance with facility policies and state and federal regulations.
Reports directly to the administrator.
QUALIFICATIONS:
1. A Bachelor’s Degree in social work, psychology or other human service field is required. One year of supervised social work experience is preferred.
2. Previous experience in a health care facility in a supervisory role or equivalent training is preferred.
4. Must have the ability to communicate effectively and tactfully to residents and family members.
5. Must be able to maintain confidentiality of resident medical, social and financial information as well as demonstrate respect for resident privacy.
6. Have the ability to work harmoniously with other departments and personnel.
7. Interest, skills and ability to work with adults in the nursing home setting.
