Social Services Director – Skilled Nursing Facility Are you a compassionate leader driven to enhance the lives of others? We are seeking a Social Services Director to join our team and ensure residents receive the social, emotional, and psychological support they need to thrive during their stay with us and beyond.

In this role, you will oversee the Social Work Department, develop comprehensive care plans, and guide residents and families through their wellness journey with dignity and respect.

What You’ll Do:

Complete social histories, assessments, and care plans upon admission to identify residents’ needs, goals, and action steps

Actively participate in interdisciplinary care plan meetings to advocate for residents’ well-being

Document timely progress notes that reflect each resident’s plan of care

Assist residents and families with discharge planning to ensure a smooth and successful transition

Lead and manage the Social Services Department for long-term and acute care residents

What We’re Looking For:

Bachelor’s degree in Social Work or a related field

Previous experience in a Skilled Nursing Facility required

Knowledge of State and Federal regulations related to social services in healthcare

Strong interpersonal and communication skills with the ability to relate positively to residents, families, and team members

Basic computer skills including email and documentation systems
