Rosemont Care & Rehabilitation Center - Rosemont Center is currently looking for a Social Services Director to join our team.

Must have some Skilled Nursing Facility Experience!

RESPONSIBILITIES OF DIRECTOR OF SOCIAL SERVICES:

Complete comprehensive social services assessments

Complete the MDS according to the predetermined schedule

Constructs a comprehensive care plan that is individualized, identifies emotional, psychological, and social needs

Meets with residents to complete various assessments

Works with other community providers to ensure each resident receives the psychosocial support necessary to enhance daily living

Complete the Pennsylvania Preadmission Screening and Resident Review (PA-PASRR) form for all new admissions

Coordinate successful discharges to the community

Coordinate and lead multidisciplinary care conferences

Advocates for each resident, including coordination of community services, hospice services, and other specialized services

Works with resident and/or resident's representative to complete POLST forms

Active participation in the Gradual Dose Reduction meeting and Quality Assurance Performance Improvement committees

QUALIFICATIONS OF THE SOCIAL SERVICE DIRECTOR:

Ability to work independently and as part of a team

Effectively communicate with residents, family members, team members, and other stakeholders

Demonstrates caring and compassion

Computer experience

Patience, enthusiasm, willingness to tackle difficult situations

Bachelor's degree in a human services field
