Responsibilities

: Oversee all clinical operations, ensuring quality patient care and adherence to regulatory standards. Lead, mentor, and develop a team of nurses, social workers, chaplains, and other hospice care professionals. Ensure the development and implementation of individualized patient care plans in collaboration with the interdisciplinary team. Monitor and evaluate patient outcomes to ensure continuous quality improvement. Provide direct support and guidance to staff, fostering a collaborative and supportive environment. Develop and maintain strong relationships with patients, families, and referral sources to promote care continuity. Ensure compliance with Medicare, state, and federal regulations, as well as company policies and procedures. Participate in budget planning and resource allocation to maintain efficient operations. Qualifications: Active Registered Nurse (RN) license in CA. Minimum of 3-5 years of experience in hospice or palliative care; prior leadership experience required. In-depth knowledge of hospice regulations, including Medicare Conditions of Participation. Exceptional organizational, communication, and leadership skills. Ability to inspire and lead a diverse clinical team with professionalism and compassion. Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite. Valid driver’s license and reliable transportation for occasional travel.
