Salary: $70,000-$90,000 (experience based)

Qualifications:
Bachelor 's Degree or equivalent experience
Minimum two years’ experience in a related health services field required
Social Worker experience preferred
Knowledge of Microsoft Office, including Word & Excel, and email applications required
Excellent written and oral communication skills
Demonstrated organizational skills with the ability to prioritize work assignments
Must communicate effectively with residents, family members and team members
Responsibilities and Expectations:
Responsible for the overall operations of the independent living spaces.
Responsible for the coordination of Independent Living services related to the 6 dimensions of Thrive Wellness including: physical, social, emotional, intellectual, vocational, and spiritual needs of residents.
Accessible to residents and family members informally as well as through planned interactions/functions.
Consults and meets with community personnel through Continuum of Care and Board of Review meetings on matters related to Independent Living residents and communicates with residents and families.
Maintains knowledge of resident conditions through the At Home Services staff, IL Health Services staff, personal visits, and involvement with resident groups, including Resident Council and family conferences.
Conducts regular staff meetings for purposes of planning, coordinating and direction of Independent Living resident services.
Coordinates processing of Independent Living resident information with administrative team.
Plans and coordinates the orientation of new residents.
Assists Executive Director with the development of the Independent Living budget.
Develops and maintains campus Independent Living policies and procedures.
Responsible for coordinating, scheduling and implementing Independent Living resident service conferences as necessary.
Ensures maintenance of the Independent Living Resident Handbook and other printed materials as well as communication through Connected Living portal.
