As the Resident Advocacy Director (RAD) at our Southpointe community, you will serve as a key support in resident care, assisting with clinical coordination, family communication, and program implementation to ensure continuity and quality of services. You will spearhead enrollment in Medicare care coordination programs, develop and implement personalized care strategies, and ensure cohesive collaboration across interdisciplinary teams and external partners. Additionally, using your unique training and experience, you will provide vitally necessary behavioral health services to residents.

Requirements
Active Licensed Independent Social Worker-Clinical Practice (LISW-CP) or Licensed Clinical Social Worker (LCSW) credentials in South Carolina
Care coordination background, case management experience, or experience as a hospice social worker
Experience delivering behavioral health interventions and psychotherapy
Preferred Skills & Attributes Comfort with electronic health record (EHR) systems and compliance audits
Understanding of Medicare care coordination programs
Demonstrated track record of data-driven process improvement

Key Responsibilities You will serve as the central care coordination specialist for every Southpointe Navion resident, revolutionizing how we deliver clinical support and partnerships.
Program Enrollment & Care Management
Enroll eligible residents in care coordination programs suitable to their specific diagnoses and person-centered care plan goals, then fulfill all program requirements and monthly duties to ensure each care plan runs smoothly and delivers the right level of support.
Partnerships & Service Coordination
Serve as the key support person and provide care coordination services for residents receiving medical, hospice, home health, therapy, and pharmacy services. Work alongside Navion’s clinical team members to manage referrals, scheduling, and documentation, while having regular communications with residents’ PCP so that every service is seamless and focused on resident needs.
Resident, Family, & Team Engagement
Build trusted relationships with residents, families, and interdisciplinary teams through regular check-ins, clear communication and proactive advocacy, ensuring preferences are honored and concerns are addressed. Support in Care Plan meetings for enrolled residents.
Community Team Member Partnership
Partner with the Director of Clinical Services, nursing team members, and Resident Care Coordinators to absorb key care coordination tasks and coach on workflows and documentation, freeing them to concentrate on direct resident care and enhancing overall team performance.
Behavioral Health & Psychotherapy
Facilitate psychosocial assessments, deliver individual and group psychotherapy sessions as needed, and align behavioral health interventions with each resident’s overall care plan.
