Position Summary
The Regional Director of Social Work and Discharge Planning covers the Greensboro/Winston Salem (Triad area) and provides leadership, oversight, and support to facility-level social work and discharge planning teams within the company. This position ensures
compliance with all federal, state, and local regulations while promoting resident-centered psychosocial
care and smooth, safe discharge transitions.
Key Responsibilities
• Indirect supervision and mentorship facility Directors of Social Services and Discharge Planners.
• Develop, implement, and maintain procedures and best practices for social work and discharge
planning.
• Ensure compliance with CMS guidelines, MDS Section Q, and all applicable state regulations.
• Audit documentation for accuracy, timeliness, and completeness.
• Oversee creation of individualized discharge plans in collaboration with nursing, therapy, dietary, and
family.
• Resolve discharge barriers including insurance issues, equipment needs, and family readiness.
• Analyze readmission trends and develop strategies to reduce avoidable hospital returns.
• Collaborate with interdisciplinary team members to coordinate resident flow and care transitions.
• Assist with complex cases involving family conflict, guardianship, abuse reporting, or grievances.
• Provide staff training to improve psychosocial care, documentation, and care coordination.
Qualification
• Bachelor’s or Master’s degree in Social Work or related field.
• Minimum 3–5 years of social work experience in long-term care, with at least 2 years in a leadership
role.
• In-depth knowledge of Medicare, Medicaid, and Managed Care requirements.
• Strong skills in psychosocial assessment, case management, and discharge planning.
• Excellent communication, leadership, and conflict resolution skills.
• Ability to travel between assigned facilities as needed.

• Reduction in hospital readmissions.
• Timeliness and completeness of discharge documentation.
• High family and resident satisfaction with the discharge process.
• Compliance with survey requirements related to social services and discharge planning
Physical Demands
This position requires the ability to sit for extended periods, travel between facilities, and occasionally lift
up to 25 pounds. Must be able to communicate effectively in person, over the phone, and via electronic
communications.
