PURPOSE OF THE POSITION The primary purpose of this role is to  integrate, coordinate, and monitor psychiatric rehabilitation services  for an assigned caseload of residents. This must be done in compliance with federal, state, and local regulations, as well as the facility’s policies and procedures, ensuring each resident’s emotional and social needs are individually met and maintained. General Responsibilities Include: Establishing a stable, therapeutic relationship with each resident.
Orienting new residents to the facility and program expectations.
Reviewing and helping residents understand their treatment plans and schedules.
Preparing residents for and supporting active participation in care plan reviews.
Coordinating and delivering psychiatric rehabilitation services.
Monitoring residents’ self-directed care and ensuring compliance with treatment plans.
DUTIES AND RESPONSIBILITIES Administrative Functions Plan, develop, organize, implement, evaluate, and direct the facility’s social services and psychiatric rehabilitation programs.
Develop and implement policies and procedures to identify and address the emotional and social needs of residents.
Complete administrative tasks such as forms, reports, and documentation, and submit to the Administrator as required.
Arrange transportation and outside appointments as needed.
Provide consultation to staff, families, and external agencies to support residents through tailored psychiatric rehabilitation programming.
Evaluate Rehabilitation Assistant performance and provide feedback and recommendations to the Administrator.
Coordinate rehabilitation activities across departments as necessary.
Collaborate with facility consultants and implement recommended changes as appropriate.
Submit oral and written reports or recommendations to the Administrator on the psychiatric rehabilitation program.
Participate in inspections and surveys by regulatory agencies.
Respond to emergencies as they arise (e.g., rescheduling work assignments or staff).
Psychiatric Rehabilitation Functions Be fully knowledgeable of psychiatric rehabilitation programs, applicable regulations, and resident rights.
Lead assessments for all new admissions and re-admissions in the assigned caseload, ensuring all documentation is complete and accurate.
Facilitate interdisciplinary care plan conferences, identifying resident needs, setting goals, and supporting discharge planning. These may include initial, quarterly, annual, or significant change reviews.
Provide individual and group counseling, behavioral interventions, and training in areas such as self-maintenance, social skills, community living, symptom management, substance abuse recovery, and vocational readiness.
Maintain current knowledge of resident needs, behaviors, and individualized care plans.
Complete accurate and timely documentation, including progress notes, monthly and quarterly summaries, IDT reviews, and chart audits. Ensure all notes are detailed, reflective of services provided, and show resident responses.
Coordinate treatment program schedules for the Rehabilitation Aide, Interdisciplinary Team (IDT), and Day Program staff.
Support the Administrator in fostering effective working relationships with all members of the IDT.
