The position coordinates the social services department to promote the psychosocial, spiritual, and emotional well-being of residents, families and staff.RESPONSIBILITIES:Possesses good communication and listening skills to identify the social, emotional, and financial needs of each short-term skilled patient and long term care residentOversees the daily functioning of the Social Service Designee.Provide services to residents/families to assist with the adjustment process and problem-solving regarding meeting social, emotional, and financial needs.Assists in the implementation of Room and Roommate ChangesAttends the Medicare Meetings or designate social service designeeCoordinates discharge planning for patients by arranging for follow up community services, home health referrals and alternative placements,Acts as an advocate for residents and families in regard to resident rights issues.Conducts in-services to educate staff as to the emotional and social needs of residents, and to teach methods for dealing with residents and families.Attends administrative staff meetings, morning report, Performance Improvement meetings, resident care-related meetings, and quarterly staff meetings.Maintains departmental records/documentation as required.Coordinates/updates resident care conference schedule or designate social service designee.Work with the front office to assist residents/families with Medicaid approval/reapplication processProvides Medicaid Pending/Patient Liability related follow-up as directed by Facility Administrator.QUALIFICATIONS:Meets the requirements for the Social Services Director for a skilled nursing facility or active social worker license in the state of the facilityLong term care experience preferred or willing to learnResident advocate within the structure of communal residential livingBS/LSW/MSW required Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
