A well-established Skilled Nursing Facility in the Bronx is seeking a dedicated, energetic, and professional Director of Social Work to join our compassionate clinical team.
Position Overview: The Director of Social Work will oversee and lead the Social Work Department and develop and implement social work programs, conduct assessments and collaborate with the interdisciplinary teams to ensure the delivery of quality care. The ideal candidate will help foster an environment that promotes dignity, autonomy, and quality of life for every resident, while contributing to a culture of respect and individualized care.
Key Responsibilities:
Conduct psychosocial assessments and develop care plans in collaboration with the interdisciplinary team.
Assist residents and families with adjustment to the facility and support during transitional periods.
Coordinate community resources and services to meet individual resident needs.
Promote a positive and inclusive atmosphere that enhances residents’ well-being.
Provide support and supervision to the Social Work Department
Qualifications:
Bachelor’s degree in Social Work (BSW) or a related human services field (required).
Prior experience working with elderly populations in long-term care or community settings (required).
Strong communication, assessment, and interpersonal skills.
Ability to work both independently and collaboratively within an interdisciplinary team.
Strong knowledge of social work principles and practices.
Previous experience in a supervisory or leadership role, preferred.
