Overview
We are seeking an experienced Director of Patient Care Services to lead clinical operations for our Cleveland-based home health agency. This role is responsible for ensuring exceptional patient outcomes, maintaining compliance with regulatory standards, and driving operational and financial performance. The ideal candidate is a strategic leader with a passion for delivering high-quality, compassionate care in a home-based setting.
Key Responsibilities
Leadership & Strategy
Direct and oversee all patient care services, ensuring alignment with organizational goals.
Develop and implement care programs to enhance quality, expand services, and improve patient satisfaction.
Build and maintain relationships with community organizations, referral sources, and healthcare partners.
Operations Management Own the P&L for the home health service line, monitoring budget performance and implementing corrective actions as needed.
Oversee the care delivery process, ensuring efficiency, timeliness, and seamless coordination.
Track KPIs, analyze data, and leverage technology (EHR systems) to optimize performance.
Compliance & Quality Ensure adherence to Medicare, Medicaid, and accreditation standards.
Conduct quality reviews and implement evidence-based practices to improve patient outcomes.
Identify and mitigate risks to patients and staff.
Team Development Recruit, train, and mentor clinical staff, fostering a culture of accountability and professional growth.
Provide ongoing feedback, recognize achievements, and support career development.
Promote collaboration across interdisciplinary teams.
Patient & Family Engagement Ensure compassionate, patient-centered care tailored to individual needs.
Educate and guide patients and families on care plans and health goals.
Represent the organization at community events and outreach programs.
Qualifications Bachelor's degree in Nursing (BSN), Doctor of Physical Therapy (DPT), Healthcare Administration, or related field required; Master's degree or advanced certification preferred.
Minimum 5 years of home health experience, including at least 2 years in a leadership role managing clinical operations.
In-depth knowledge of OASIS, Medicare/Medicaid regulations, and accreditation requirements.
Proven ability to lead interdisciplinary teams and drive operational and clinical excellence.
Strong organizational, communication, and problem-solving skills.
Proficiency with EHR systems (e.g., HomeCare HomeBase).
Work Environment Office-based with frequent field visits to oversee patient care, support staff, and engage referral sources.
Fast-paced, patient-focused setting requiring adaptability and decisive leadership.
Local travel required within the service area.
