This includes evaluating residents, identifying areas of concern and coordinating services with the interdisciplinary team as well as contacting community resources as needed. The Director of Social Services will have a strong managerial background as he/she will oversee case management services for residents and review resident status.Requirements of the position:Required Education and ExperienceBachelor's Degree in social work (SW) or social welfare from an accredited academic institutionAt least 2-years experience in health care settingExcellent written and verbal communication skillsComputer proficiency with Microsoft productsPreferred Education and ExperienceCurrent state license as a Social WorkerExperience in outpatient rehabilitation preferredExperience in a fast-paced environmentPost-acute care or health care experience preferred#Watertown Health Care CenterCome in for an interview!
