Healthcare Nursing Center - A respected and growing long-term care organization is seeking an experienced and compassionate Director of Social Services to lead our facility’s social work department. This individual will play a key leadership role in supporting residents and families through the transition to long-term care and ensuring that psychosocial needs are met with professionalism, empathy, and excellence.
Responsibilities:
Oversee all aspects of the Social Services Department, ensuring compliance with state and federal regulations.
Conduct comprehensive psychosocial assessments and develop individualized care plans in collaboration with the interdisciplinary team.
Provide counseling, support, and advocacy for residents and families during admission, discharge, and throughout their stay.
Lead and mentor social services staff to maintain high-quality standards of care and service.
Collaborate closely with nursing, therapy, and administrative leadership to promote resident well-being and satisfaction.
Maintain accurate documentation and ensure timely completion of required reports.
Qualifications:
Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.
Current Social Work licensure as required by state regulations.
Minimum of 3 years’ experience in a skilled nursing or long-term care setting, with at least 1 year in a supervisory or leadership capacity.
In-depth knowledge of discharge planning, psychosocial assessments, and community resources.
Exceptional communication, organization, and interpersonal skills with a resident-centered approach.
