Job Summary
Our client is seeking a Social Services Coordinator for various programs including CalAIM and internships. This full-time on-site position involves coordinating daily operations of programs, providing leadership and guidance to staff, and ensuring compliance with policies and procedures.
Responsibilities / Job Duties
Manage and coordinate daily operations of CHTC Programs, providing supportive leadership and guidance to staff.
Collaborate with departments to ensure seamless care and efficient service delivery.
Conduct comprehensive assessments and develop care plans with input from clients and the care team.
Maintain positive relationships with clients to meet their needs and achieve high satisfaction.
Conduct outreach to potential members and coordinate a warm handoff to CHTC Care Managers for ongoing care.
Ensure documents are completed accurately and in a timely manner.
Monitor and evaluate the effectiveness of the CHTC's Programs and services.
Analyze data and report program performance to senior management.
Identify and recommend strategies to improve client outcomes and satisfaction.
Ensure ongoing compliance with state regulations and company policies.
Lead regional growth of existing CHTC Program teams and expand the program with new clients.
Identify KPIs that align teams with company goals and ensure monthly performance standards are met.
Inspire team members and foster a positive, diverse, and engaging team-oriented culture.
Support outreach processes to achieve or exceed established goals.
Provide consultation, case management services, and guidance to ECM and Community Support Program clients.
Supervise Care Managers, including staff and interns, and provide support to Program Managers (LCSW).
Minimum Education and Experience Qualification Requirements
Education Master's degree in Healthcare, Social Work, or a related field.
Qualifications Strong focus on accountability and building positive relationships with clients to ensure high-quality care.
Excellent leadership, supervision, and team management skills.
Experience in program management and strategic planning.
Experience Preferred experience with providing services in areas such as homelessness, unstable housing, substance use disorders, mental health issues, abuse, and incarceration.
Experience with Medi-Cal Transformation (CalAIM), Enhanced Care Management (ECM), or Community Supports Services.
