What You’ll Do:
Coordinate the full admission and transfer process for residents, ensuring smooth transitions and positive experiences
Lead the Social Services department, supervising and developing team members
Complete and maintain social service assessments, care plans, and MDS documentation
Provide support and advocacy to residents and families to enhance well-being and quality of life
Serve as the abuse and neglect coordinator, ensuring compliance with state and federal regulations
Collaborate with interdisciplinary teams to drive quality and service excellence
Schedule:
Full-time, salaried position
Day hours with weekend rotation required What You’ll Need:
Licensed Clinical Social Worker (LCSW) certification required
Minimum 3 years of experience in a Skilled Nursing Facility (SNF) with 100+ beds
Strong leadership, communication, and organizational skills
