Heritage Skilled Care - Job Summary: The Director of Social Services is responsible for leading and managing all aspects of social services within the assisted living facility. This includes assessing residents' psychosocial needs, developing individualized care plans, providing emotional support, coordinating with families, and ensuring compliance with applicable regulations. The Director plays a key role in enhancing the quality of life for residents through advocacy, resource coordination, and ongoing support.
Key Responsibilities:
Resident Assessments & Care Planning
Conduct initial psychosocial assessments for new residents.
Participate in the development and review of individualized service plans (ISPs).
Monitor residents’ social, emotional, and mental well-being.
Resident and Family Support
Provide counseling and support to residents and families during transitions and challenging times.
Act as a liaison between residents, families, and facility staff to resolve concerns and promote well-being.
Facilitate family meetings as needed.
Program Development & Implementation
Develop and implement programs and services that support residents' social and emotional needs.
Coordinate with activity staff to ensure psychosocial needs are incorporated into programming.
Staff Training & Support
Train and support staff on topics such as dementia care, grief support, communication techniques, and resident rights.
Provide guidance to care teams regarding behavioral interventions and crisis management.
Regulatory Compliance & Documentation
Maintain accurate and confidential resident records in compliance with HIPAA and other applicable laws.
Ensure compliance with federal, state, and local regulations.
Participate in audits, inspections, and quality improvement initiatives.
Community Resource Coordination
Connect residents and families with appropriate community services, such as counseling, legal aid, and financial assistance.
Build and maintain relationships with external agencies and service providers.
Qualifications:
Education: Bachelor’s or Master’s degree in Social Work (BSW or MSW), Gerontology, Psychology, or a related field.
Experience: Minimum of 3–5 years in a social services role within a senior living, long-term care, or healthcare setting; supervisory experience preferred.
Licensure: Licensed Social Worker (LSW or LCSW) preferred, as required by state regulations.
Skills:
Strong interpersonal, communication, and crisis intervention skills.
Knowledge of aging-related issues, including dementia, elder abuse, and end-of-life care.
Familiarity with state and federal regulations governing assisted living facilities.
Work Environment: Work is performed in a residential assisted living setting.
May involve occasional evening or weekend hours to accommodate resident or family needs.
Requires occasional lifting, standing, or movement in a clinical environment.
