FT MSW Social Worker DUTIES: • Complete clinical assessments and social service documentation as required by state and federal mandates and facility policy. • Demonstrate effective practice skills, which integrate social work theory, regulations and sound working relationships to enhance positive outcomes for patients and families. • Interviews residents and their family members to solicit information regarding the resident’s strengths, weaknesses, coping strategies and lifestyle prior to placement and develops and initial psychosocial assessment along with a detailed social history within the clinical record, • Maintains and improves resident’s ability to control everyday physical needs and mental/psychosocial needs • Identify individual social and emotional needs • Develops and maintain resident psychosocial assessments and quarterly/routine care plans, • Educates residents, family members, and / or significant others regarding advance care planning and community resources, • Works with the interdisciplinary team to assure resident and family concerns are addressed in a timely manner. Advocates for the needs of residents in ensuring resident rights, • Educate residents, families, and staff on resident rights, resident abuse, mistreatment and neglect regulations to ensure residents are protected.  Participate in orientation process with staff developer. • Provide information and documentation to residents and families with regards to access to Medicare, MassHealth and financial and insurance benefits. • Collaborates with social service team members to ensure the needs of residents are met with professionalism, dignity, while providing resident choice. • Educate resident and families about palliative care and end of life choices. • Participates in the planning of the resident’s admission, community re-entry, transfer to another facility and discharge planning.

REQUIREMENTS: Master’s in social work/ (MSW) Long-term care/ sub-acute experience required
