MVP Recruitment is proud to partner with a highly regarded Skilled Nursing and Rehab Center in the Westwood MA area to recruit a compassionate and experienced Director of Social Services. This full-time leadership role will support the Social Work department, with a primary focus on the Short-Term Rehabilitation Unit. The ideal candidate will bring strong clinical knowledge, excellent communication skills, and a passion for supporting patients and families through transitions of care. Position Overview: The Director of Social Services is responsible for leading the daily operations of the Social Services department, ensuring high-quality psychosocial care and effective discharge planning for residents—particularly those in short-term rehab. This role works closely with the interdisciplinary team and plays a key part in care coordination, family communication, and regulatory compliance. Key Responsibilities: Supervise and support social work staff across the center

Conduct and oversee psychosocial assessments, care planning, and discharge coordination

Serve as the primary liaison between residents, families, and the clinical team

Coordinate post-discharge services such as home care, outpatient therapy, or long-term placement

Address psychosocial concerns, grievances, and provide counseling as appropriate

Ensure accurate and timely documentation in accordance with state and federal guidelines

Participate in interdisciplinary meetings and quality improvement initiatives

Provide education, mentorship, and performance oversight for the social work team

Qualifications: Bachelor’s or Master’s degree in Social Work (BSW or MSW)

Current MA LCSW or LICSW preferred

Minimum of 3 years of experience in a Skilled Nursing or Post-Acute Rehab setting

Prior supervisory or management experience preferred

Strong understanding of discharge planning and community resources

Excellent communication, documentation, and organizational skills
