Description Director of Social Services needed for full-time, temp to hire position in Philadelphia.Responsibilities include conducting assessments for each resident, identifying social needs and goals in written care plans, and involving residents and families in care planning. The social worker will report concerns to the Director of Nursing or Administrator, complete social service sections of MDS assessments promptly, and review nursing notes to ensure care plan compliance. This role also includes discharge planning, maintaining quality customer service, and supervising other social workers in the department.Position Requirements MSW or PA LSW.
