Job Details Job Location : Albany, GA Position Type : Full Time Salary Range : Undisclosed Description Job Summary: Manage daily program operations for the Residential Services and Day Services Departments including budget management and control, individual services in all residential and day programs and personnel management. Job Duties & Responsibilities: Manage program operations of all residential and day services. Insure all services remain in compliance with all applicable standards and regulations. Work with Residential Managers, Support Coordinators, Medical Providers, Social Security, DFCS, Adult Protective Services, Mental Health, and other Service Providers to provide optimal individual care. Insure services follow the high standards ESSG program lines are known for that contribute to the daily lives of the individuals we serve. Monitor staffing for the department to address staffing needs and collaborate with HR to screen, interview and select qualified staff. Insure that staff is in place to provide all approved services for each location as scheduled to insure labor is managed within appropriate budgets. Insure that all staff receives proper introductory training and completes continuing education requirements. Insure that all staff maintains current required certifications including CPR, First Aid, Defensive Driving, Professional Certification and Licensing, and TB clearance. Schedule, plan and conduct monthly team meetings. Manage work schedules, duties and responsibilities for program managers. Provide on-going, effective performance management to all staff including progressive discipline, annual evaluations and goal planning and review. Evaluate the performance of staff to ensure that services are of appropriate quality and that resources are used effectively. Assist managers as needed with performance management of staff. Operate program within projected budget to insure program efficiency while seeking and recommending potential opportunities for cost containment and anaylze variance from budge or anticipated trends. Work with managers and finance staff to identify, create and submit Additional Residential Staffing and Renewals as needed for additional funding and to insure appropriate funding for each program location. Collaborate with President/CEO and program managers for intake and evaluation of potential clients and other program locations. Conduct visits to all properties for scheduled and unannounced review of operations. Assess training needs and provide information to responsible parties to insure compliance. Monitor and insure individuals health, safety, development and quality of care and services. Maintain contact with client’s families, support coordinators, funding sources and other parties involved in clients’ services as needed and required. Insure client satisfaction by assisting all in a timely and considerate manner. Receive and return phone calls, greet and assist individuals, families and others desiring service information and to address concerns as they arise in a timely manner. Inspect, prepare and submit monthly reports within the established deadline for service billing. Insure that all necessary documents (history, medical information, doctor’s notes, referral forms,care plans, etc.) are maintained in case files in a timely manner. Review electronic documentation for each individual as needed. Monitor and insure appropriate use of individuals funds and personal spending money per individual's requests and budget allowances. Support individuals in personal growth and development while respecting differences. Insure all individuals rights are appropriately respected and honored as well as report any violations. Review individual goal tracking to ensure that all services are provided and documented in accordance with individual ISP when applicable. Insure submission of required documents to various departments as scheduled. Manage and insure compliance of all individual documentation and records. Complete or review Critical Incident Reports as necessary and submit within established guidelines. Review individual satisfaction surveys & implement changes when feasible. Approve and submit travel reimbursement for managers for all travel. Complete continuing education as required for growth and development. Participate and engage with the ESSG management team for meetings, future planning and collaboration. Serve as a key player in strategic planning for the organization, with a focus on the residential and day service programs. Follow Health Insurance Portability and Accountability Act (HIPAA) privacy rule to insure individual protected health information is secure. Maintain knowledge and understanding of Easterseals Southern Georgia, Inc. policies and procedures. All other duties as assigned. DDP Duties & Responsibilities: Overseeing the services and supports provided to participant for general guidance to the provider agency in areas of compliance and quality improvement; Assuring that the supports provided are within the scope of the agency’s service enrollment and experience to assure effective delivery; Assuring that the services address the participant’s needs and adhere to the application of person centered values, choice and participant’s rights; Providing, arranging or overseeing curricula used in staff training and directed to service delivery in the context of the individual’s goal(s) and objectives; Recommend other needed services/supports or changes to the delivery model using a continuous quality improvement approach; Providing consultation to the provider agency in ISP implementation strategies that are specific,measurable, achievable, relevant, realistic and time limited in order to meet the needs and personal goal(s) of the participant; Assess areas of risks either individually or overall risks to persons supported through agency practice,policy or lack of policy or procedures/protocols. Providing risk mitigation strategies to the provider agency; Reviewing that functional assessments are in place to support formulation of the participant’s plan for delivery of all waiver services that include: The Health Risk Screening Tool; The Supports Intensity Scale; Functional Behavioral Evaluation; Others (E.g., Nursing, OT, PT etc.) as needed or required.
Oversee high intensity services if applicable that address health and safety risks for the participant’s that includes: The implementation and effectiveness of Behavior Support Plans; The implementation and effectiveness of the Participant’s Crisis Plan; and Identifying ongoing supports as needed (medical and /or behavioral) in collaboration with agency staff, staff of other agencies providing supports to the participants mutually served or other members of the healthcare team. The provision of DDP oversight must be documented in the individual’s record no less than quarterly. This documentation should include: all necessary face-to-face individual visits, other contact, or communication with or on behalf of the individual. the purpose of the visit or contact, assessment or evaluation, training, plan for intervention, and any changes in service delivery such as change in staff recommendations.
Essential Functions: 24 hours a day / 7 days weekly availability for emergency situations. Travel to multiple ESSG properties and community locations for individual management, meetings and events,with travel up to 200 miles that may warrant occasional overnight stays within our geographic territory. Ability to network within the community to create and maintain relationships for residential and day services program needs and development. Ability to lift, push and pull a minimum of 50 pounds. Ability to bend, stoop and physically maneuver to properly secure individuals and wheelchairs in vans for transport as needed. Ability to speak publicly for various groups to advocate for ESSG mission, vision, values and clients. Use of technology and office equipment including: laptop, email, Smartphone, Microsoft Office Suite,scanner, fax, printer, PolicyStat system, THERAP system, Provider Connect System, Health Risk Screening Tool (HRST) and other individuals records management systems. Job Specifications (Characteristics to perform successfully): Extensive knowledge of residential and day services and developmental disability services in Georgia. Exhibit a high standard of leadership to effectively manage the department’s services. Require and insure a high standard of individual services from departmental team. Ability to effectively multi-task and execute expectations from different departments. Maintain objectivity in position in order to set appropriate limits while working with individuals and employees. Work cooperatively and responsively with ESSG Leadership to ensure optimal care of individuals. Develop and maintain positive and productive relationships with Individuals, Families, Co-Workers and Community Members. Ability to manage time effectively and exhibit organizational skills. Support individuals in personal growth and development while respecting differences. Qualifications Minimum Qualifications: Bachelor's Degree in Social Services, Education, Sociology, Psychology or related field of study, Master’s degree preferred. 5 years professional work experience in disability services. 5 years professional work experience in management including departmental budgeting and development. Extensive knowledge of state requirements and guidelines for residential services. Valid Driver’s License. Pass background checks including: motor vehicle records check, criminal background check, preemployment physical and drug screening.
