About the Role We are seeking an experienced and strategic Director of Pharmacy to lead all pharmacy operations for an innovative, home-based acute and post-acute care program. This role is responsible for ensuring safe, effective, and compliant medication management across home care settings, bridging clinical, operational, and logistical functions to support high-quality patient outcomes.

Key Responsibilities Oversee clinical pharmacy activities, including IV infusions, anticoagulation, pain management, and post-surgical meds. Manage medication logistics: procurement, storage, delivery, and inventory for home care programs. Ensure compliance with state/federal pharmacy regulations and accreditation standards. Integrate pharmacy systems with EMRs and remote monitoring platforms. Collaborate with medical, nursing, and operations teams to optimize patient outcomes. Lead, mentor, and grow a team of pharmacists and support staff. Track performance metrics: adherence, adverse events, and readmissions.

Qualifications PharmD or equivalent degree; active pharmacist license in applicable state(s). 8+ years of pharmacy experience, with at least 3 years in a leadership or management role. Inpatient care experience. Experience in acute care, post-acute care, or home-based healthcare preferred. Strong knowledge of pharmacy regulations, controlled substances, and compliance standards. Demonstrated success managing complex logistics and operational challenges. Excellent leadership, communication, and cross-functional collaboration skills.
