Work Shifts 9:00 A.M – 5:00 P.M Duties & Responsibilities Purpose of Position: The Director of Quality Management reports directly to the Chief of Strategy, Quality, and Safety and serves as an integral member of the Community Care leadership team. The Director is responsible for overseeing all quality management activities across Community Care programs, including home- and community-based services, care coordination, and population health initiatives. The role formulates and implements effective strategies, policies, and performance improvement initiatives that ensure compliance with governing and accrediting bodies, enhance patient safety and strengthen the overall experience of patients and families. By aligning quality outcomes with organizational strategy, the Director ensures that Community Care delivers high-reliability, patient-centered, and cost-effective services to the populations it serves. Essential Duties and Responsibilities: 1. Directs the overall day-to-day operations of the Quality Management Department’s activities following evidence-based guidelines and regulatory requirements and is accountable for the operational excellence and quality delivery of service across Community Care. 2. Designs, develops, and implements effective departmental strategies and programs throughout Community Care focused on improving quality, patient experience, and population health outcomes while reducing the cost of care. 3. Partners with key stakeholders to advance departmental initiatives in alignment with the strategic direction defined by the Chief of Strategy, Quality, and Safety. 4. Monitors the effectiveness of the department’s activities and quality management programs across Community Care and facilitates successful evaluation of quality standards in line with governance provided by external accrediting bodies. 5. Coordinates the collection and analysis of quality data and utilizes findings to drive improvements, mitigate risks, and elevate patient care. Oversees the accuracy, integrity, and timeliness of quality data reporting and required submissions to payers, regulatory bodies, and accrediting agencies. 6. Recommends organizational objectives related to strategic plans, establishes service goals, and identifies issues affecting the quality of care. Implements service and process improvement initiatives, utilizing established quality improvement methodologies (e.g., PDSA, Lean Six Sigma) to monitor performance, evaluate initiatives, and drive continuous improvement. 7. Serves as a key resource to senior leadership, clinical staff, and other stakeholders on quality management techniques and systems. Presents findings of assessments, analyses, and safety incidents to executives and/or senior managers. Advises on upcoming regulatory changes and required actions needed to meet or exceed new industry standards and achieve improved outcomes. 8. Provides leadership, supervision, and professional development opportunities for quality management staff, including managers and assigned personnel. Ensures effective execution of chart reviews, audits, data abstraction, and quality reporting activities, while overseeing the accuracy, integrity, and timeliness of submissions. 9. Supports quality governance processes by coordinating materials, preparing agendas, reports, and presentations, and tracking follow-up for quality and safety committees and leadership forums. 10. Routinely assesses staff learning needs and implements a competency assessment program to identify gaps. Evaluates the quality and effectiveness of programs and updates training to maximize productivity, effectiveness, and efficiency. 11. Responsible for oversight of all processes necessary to meet regulatory standards from governing bodies and accrediting agencies. Maintains readiness for unannounced agency visits and participates in surveys. Serves as regulatory liaison, specifically as it relates to departmental performance and the creation of action plans. 12. Investigates incidents involving patient care and safety, and makes reports to appropriate public health and regulatory agencies. Presents findings to relevant parties, incorporates lessons learned into procedural tools and programs, and makes recommendations to prevent recurrence and strengthen the culture of safety. 13. Prepares and/or actively participates in the development of the quality management fiscal year budget and allocation of funds. Ensures operations run within budgetary guidelines, reviews requests for quality initiatives, and identifies and meets staffing needs to support the continuity of care. 14. Performs other related duties, as directed. Minimum Qualifications 1. Licensed and currently registered to practice as a Professional Nurse in New York State; and, 2. A Masters Degree in Nursing Administration or an equivalent field, and a minimum of four years of high-level responsible experience in nursing administration, three years of which shall have been in a hospital; or, 3. A satisfactory equivalent.  However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State. Department Preferences Purpose of Position: The Director of Quality Management serves as an integral part of the multidisciplinary team overseeing the department’s activities across the health care setting directing and managing the daily operations of the department. The director formulates and institutes effective strategies and policies that focus on the delivery of quality, cost- effective health care services. Ensures optimal utilization of resources and compliance with the operating standards of various health care governing and accrediting bodies, resulting in an improved patient experience. Essential Duties and Responsibilities: 1. Directs the overall day-to-day operations of the Quality Management Department’s activities following evidence-based guidelines and regulatory requirements and is accountable for the operational excellence and quality delivery of service across the health care setting. 2. Designs, develops, and implements effective strategies and programs throughout the health care setting focused on improving quality, patient experience, and population health outcomes and reducing the cost of patient care. 3. Partners with key stakeholders to define the strategic direction for the Quality Management Department’s initiatives, ensuring alignment with the System’s mission, values, and business goals. 4. Monitors the effectiveness of the department’s activities and all quality management programs across the health care setting and facilitates successful evaluation of health care quality standards in line with governance provided by external regulatory agencies, as well as by other entities. 5. Coordinates the collection and analysis of all data related to quality indicators and utilizes findings to drive improvements, mitigate risks, and to increase the level of care within the System. 6. Recommends organizational objectives related to strategic plans and establishes service goals and identifies problems affecting the quality of service. Implements service and process improvement initiatives and other programs utilizing cost-effective solutions and service delivery enhancements. 7. Serves as both principal advisor and key resource to senior leadership, clinical staff, and other appropriate parties on matters pertaining to quality management techniques and systems, and presents findings of assessments, analyses, and safety incidents to various agencies, executives and/or senior managers. Advises on upcoming regulatory changes and required actions needed to meet or exceed new industry standards and to achieve improved outcomes. 8. Deploys department resources in a strategic manner with the goal of achieving excellence in service delivery. Provides ongoing quality management training, guidance, and developmental opportunities to staff who impact the delivery of care. 9. Routinely assesses staff learning needs and implements a competency assessment program to identify gaps and evaluates the quality and effectiveness of the programs. Implements necessary changes in trainings to maximize productivity, effectiveness, and efficiency of staff. 10. Responsible for oversight of all processes necessary to meet regulatory standards from various governing bodies and accrediting agencies. Maintains readiness for unannounced agency visits and participates in various regulatory agency surveys. Serves as regulatory liaison, specifically as it relates to departmental performance and the creation of action plans to assist leadership in communicating newly adopted approaches in reaching medical excellence. 11. Investigates incidents involving patient care and safety, and makes reports to the appropriate public health and regulatory agencies. Presents findings to related parties and incorporates new findings into affected procedural tools or programs. Makes recommendations relating to the prevention of such incidents and to determine quality control measures to foster a culture of safety for patients, staff, and visitors. 12. Prepares and/or actively participates in the development of the quality management fiscal year budget and allocation of funds. Ensures operations run within budgetary guidelines, reviews budget requests for quality management initiatives, identifies and meets staffing needs to support the continuity of care. 13. Performs other related duties, as directed.
