Responsible for meeting the goals and objectives set forth by ACLC, including plan, support, and related deliverables.Responsible for planning, administration and monitoring of Ambulatory processes and systems in accordance with hospital policies and procedures, The Joint Commission Standards and other regulatory agenciesMeasures and monitors quality management activities through collection and analysis of data and communicating these activities within the ACLC DepartmentResponsible for the quality assurance, regulatory, and professional Nursing standards of care. Identifies issues and concerns regarding clinical documentation and practices reflected therefore Enhance quality of care management through a systematic performance improvement process and staff education Collaborates with department leadership to identify recommendations for high quality, cost effective care and practice improvements, based on areas identified in analysis of Quality Indicator dashboard and performance improvement measuresProvides guidelines and support to the Ambulatory and departmental leadership in development of performance improvement projects.Acts as resource to staff in initiatives (depression, diabetes and hypertension, accountable care organization) in accordance with evidence -based practice guidelines in Primary Care.Participates in PCMH programs routinely to ensure compliance with new changes and standardsEnsures that monitoring and evaluation of the standards of care and practices are adhered to whenever there is a patient occurrence and provide feedback to nursing leadership and staff development instructorsProvides recommendations in providing staff education for any identified issues or updates in Ambulatory practices. Assists in the responsibility for communication and education regarding activities in the Ambulatory practices, and serves as a resourceHelps coordinates and direct activities related to The Joint Commission (TIC) accreditation and continuous readiness, and in collaboration with hospital Quality and Risk, to ensure that TJC deficiencies are addressed and resolved. Collaborates with Quality/Risk Management Department, QAPI Committee, The Joint Commission (TJC) Readiness, Infection Prevention.Develops and follows through on Nursing Engagement Improvement PlanParticipates in peer review meetings, and reports out any issues related to Ambulatory practices.Continually develops skills and knowledge related to Ambulatory, leadership and change management through formal and informal training literature review, participation in conferences and professional organizations.Demonstrates knowledge of evolving and best practices related to the applicable clinical area and related performance improvement and patient safetyReviews and/or revises Policies and Procedures as neededDemonstrates managerial competency in working collaboratively and relating well with other department/disciplinesFacilitates communication and teamwork with other disciplines and departments to resolve and prevent problem Responsible for facilitating staff participation in satisfactory/engagement surveyProvides feedback/recommendations as needed for improved efficiency and accuracy for clinical documentation practices Effectively collaborates with stakeholders of alt levels of staff, developing trusting working relationships with leadership, physicians, and front-line personnel. In leadership or supporting roles; actively identifies opportunities to informally mentor department staffResponsible for fostering innovation and excellence in nursingCollaborates and implements new initiatives from Central officeCoordinates and supports other activities related to professional nursing practice and care innovation as assigned by the Chief Nursing OfficerPartners with all nurse leaders to ensure focus on exemplary professional practice, quality, safety and patient care outcomesPerforms other duties as assignedMinimum Qualifications1. Licensed and currently registered to practice as a Professional Nurse in New York State; and,2. A Master's Degree in Nursing Administration or an equivalent field, and a minimum of four years of high-level responsible experience in nursing administration, three years of which shall have been in a hospital; or,3. A satisfactory equivalent. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.Department PreferencesCurrently registered to Practice as a Professional Nurse in NY State plus (2) or (3) below NY Registered Nurse (RN) license Master's Degree in Nursing.3+ years of Nursing experience in Ambulatory Care.ANCC Board Certifications in Ambulatory Practices.Extensive knowledge of the American Nurses Association Standards of Practice, The Joint Commission Resources, NationalPatient Safety Goals, Patient Bill of Rights, Department of Health Standards and Hospital Policy and ProceduresExcellent communication (both oral and written), interpersonal, organization, presentation skills Great interpersonal skillsStrong attention to detail Highly organized, ability to work independentlyLeadership Skills: Effective leadership abilities as demonstrated by:Ability to implement change in a positive, sensitive and forward-thinking mannerDeveloping goals and objectives, and establishing prioritiesInspires confidence, appropriate risk taking and achievement of high standardsSelf-starter with a willingness to try new ideas.
