If this sounds like you, we'd love to connect.Position SummaryThe Director of Nursing and Clinical Services leads nursing operations in our 54-resident Skilled Nursing community and provides clinical guidance to our Assisted Living and Memory Care neighborhoods, which include 38 apartments.In collaboration with the Administrator, the Director of Clinical Services fosters a culture of accountability, collaboration, and excellence, aligning clinical and operational priorities to support census, budget, and resident experience goals. This visible leadership role blends operational and relational responsibilities, engaging consistently with residents, families, and team members. As a key clinical leader, you will guide care across all service areas, ensuring compliance with regulatory standards, optimizing reimbursement models, and strengthening care delivery practices. Demonstrate and expect ethical behavior and professionalism, serving as a resource and mentor for nursing staff at all levels. Model Company culture across all areas including skilled nursing, Assisted Living and Frasier Care, setting high standards and expecting the clinical team in all service areas to do the same. Adept at aligning nursing operations with long-term organizational goals. Lead and develop a high-performing clinical team, encouraging growth, fostering a culture of accountability, collaboration, trust, and excellence. Model and uphold Frasier's core values in all interactions with residents, families, and team members. Partner with the Administrator to align clinical and operational strategies, actively contributing to census growth, budget performance, and resident satisfaction through data-driven and relationship-centered leadership. Clinical & Operational Oversight Identify inefficiencies in clinical workflows and implement evidence-based protocols to streamline care delivery while maintaining compliance and resident safety. Ensure delivery of safe, high-quality, person-centered care that meets or exceeds regulatory standards. Oversee clinical operations, including care planning, infection control, and quality improvement initiatives. Support the management of clinical policies and procedures by ensuring timely review, updates, and alignment with regulatory standards; lead or coordinate education and re-education of clinical team members to ensure understanding and consistent application in daily practice. Optimize use of electronic health records and nursing software (Point Click Care) to reduce redundancy, improve reporting accuracy, and support real-time clinical decision making. Strengthen hand-off communication process between departments and during transitions of care (e.g., hospital to SNF) to strengthen care pathways and improve continuity of care. Manage Health Services on-call rotations and ensure continuity of care across shifts. Maintain up-to-date emergency response protocols and ensure staff training on safety procedures. Team Development & Performance Management Lead, recruit, mentor, and develop clinical staff while fostering a culture of engagement, professionalism, accountability, and continuous growth. Provide regular coaching, feedback, and performance evaluations, implementing performance improvement plans, when necessary, setting clear expectations and holding team members accountable for excellence. Address staff concerns, conflicts or grievances promptly and professionally to maintain a cohesive work environment. Demonstrate empathetic leadership by listening to staff, addressing concerns, and partnering with them to achieve goals. Lead competency-based training programs to ensure team readiness and adherence to clinical standards and best practices. Strategic & Financial Management Collaborate with leadership to meet census, financial, and staffing goals through effective resource utilization and cost containment strategies. Analyze clinical, financial, and operational data to identify trends, address challenges, and implement measurable improvements. Maintain appropriate staffing levels to support both resident needs and team sustainability, balancing quality with financial responsibility. Assess staffing models and skill mix to improve staff deployment, reduce overtime and enhance care consistency across shifts. Regulatory Compliance & Quality Assurance Maintain expert knowledge of state and federal regulations, ensuring compliance and readiness for surveys. Provide leadership and oversight during annual, complaint, and follow-up surveys; ensure clinical documentation and medical records meet regulatory standards, are audit-ready, and reflect accurate, timely, and complete care delivery. Collaborate with Medical Records and interdisciplinary teams to maintain ongoing survey readiness. Oversee risk management initiatives, including incident reporting, safety protocols, and infection control programs. Lead Quality Assurance and Performance Improvement (QAPI) initiatives, using data driven analysis to monitor trends, and driving continuous improvement in clinical outcomes. Resident & Family Experience Lead initiatives that improve the experience of care through personalized care plans, resident input and service delivery enhancements Serve as a visible and trusted clinical leader for residents and families, proactively addressing questions and concerns. Foster open communication and partnership with families, residents, and the interdisciplinary team to ensure continuity and person-centeredness in care planning. Promote dignity, respect, and inclusion in all aspects of the resident and family experience, ensuring care is tailored to individual preferences and needs. Here's what you'll need:Licensed Registered Nurse (RN) with an active, unencumbered license (ADN required, BSN required, and MSN or related advanced degree preferred) 5+ years of progressive nursing leadership and management experience Deep understanding of skilled nursing, long-term care, or senior living settings Demonstrated success managing clinical operations across multiple service lines, with a strong focus on regulatory compliance, quality outcomes, and resident experience Proven experience leading interdisciplinary teams and implementing Quality Assurance and Performance Improvement (QAPI) initiatives Strong background in budget management, census development, and resource allocation to support operational and financial goals Deep understanding of state and federal regulations (CMS, OSHA, CDC) and survey readiness requirements.Proficiency in Electronic Health Records (EHR) systems (experience with PointClickCare (PCC) is a plus). Knowledge of Medicare/Medicaid reimbursement models, including PDPM. Experience managing infection control programs and implementing evidence-based clinical practices. Compensation and BenefitsPay Range: $140,000-$160,000 per year. Bonus eligible.
