The Clinical Director will play a crucial role in supervising and training primary care providers (PCPs) in the assigned center. This leadership position is responsible for enhancing clinical quality, efficiency, patient outcomes, and overall satisfaction among clinicians and patients. A portion of your time will also be dedicated to direct patient care as a PCP, with responsibilities shared based on the needs of the center and clinical team.
Key Responsibilities :
Provide independent care to patients, managing both acute and chronic illnesses typical in older adults.
Be accountable for patient care outcomes and collaborate with specialists as necessary while maintaining oversight of the patient's health care.
Interact with hospitalists regarding hospitalized patients, ensuring smooth transitions of care.
Conduct assessments, diagnoses, treatments, management, education, health promotion, care coordination, and documentation for complex health needs.
Lead a care team of medical assistants, facilitators, and coordinators to ensure comprehensive patient care.
Collaborate with transitional care teams for patients in various settings, including hospitals and skilled nursing facilities.
Facilitate weekly Super Huddles and Transforming Care Meetings, providing guidance and support for the center as needed.
Actively engage in managing the center and providing coverage for absent providers, while also playing a role in patient recruitment and interviewing new team members.
Perform additional duties as required by management.

Qualifications :
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty is required.
Must have a current and active MD license in the state of employment.
A minimum of 1 year of clinical experience in a geriatric, adult, or family practice setting is preferred; candidates with ChenMed experience are encouraged to apply.
Completion of ChenMed training required.
Board certification in a relevant specialty is preferred; board eligibility is required.
Basic Life Support (BLS) certification is mandatory.
A valid DEA number for controlled substances (schedule II-V) is necessary.
