Job Details

Leadership role overseeing 3 full-time case managers, 1 assistant, and 2–3 per diem staff Team includes RNs, a Physical Therapist, and Social Workers Case managers carry a manageable caseload of 12–13 patients

Requirements RN with BSN or Social Worker with MSW or Social Work License CCM or ACM certification required within first year of hire Strong leadership and team management skills Experience in case management preferred
