: Director of Case Management Location: Nashville, TN Position Summary
The Case Management Division Director is responsible for supporting the execution of the case management program across the division. This position provides direct onsite support and oversees change management initiatives. Key responsibilities include data analysis, facility gap assessments, and action plan development to achieve established goals. The Division Director also directs facility case management regarding care coordination and transition processes, ensuring compliance with standards, policies, and regulatory requirements. Responsibilities Provide leadership and actively participate with facility staff to identify, implement, or enhance Case Management programs consistent with HCA and facility strategy. Disseminate information accurately and timely with multiple audiences; lead regularly scheduled communication sessions with facility CM Directors. Identify process improvement opportunities; develop, implement, monitor, and revise action plans. Identify potential best-demonstrated practices to explore implementation. Identify opportunities to establish standards and policies. Collaborate with key stakeholders at corporate and division levels to explore development. Facilitate Case Management education within the division; assist with orientation and ongoing education of Case Management staff. Identify and propose opportunities to improve current technology or identify new technology to enhance performance and productivity. Education & Experience Bachelor's degree required; Master's degree in Nursing, Health Administration, or Business Administration preferred. 5+ years of recent acute care experience in Case Management Leadership within a large multi-hospital system (Director level or higher). 7+ years of overall Clinical Case Management experience. Registered Nurse - Currently licensed as a registered professional nurse in the state of practice. Certification in Case Management required. Additional Qualities Desired in Candidate Corporate polish and excellent communication skills. Proven motivational capabilities. Executive presence. Demonstrated record of accomplishment for delivering results and attaining goals. Culture fit for HCA.
Pay Range: $125,800 - $160,000 per year
