Director of Case Management (RN) Location: Ottumwa, IA
Schedule: Full-Time | Day Shift | 7:00 AM - 3:00 PM
We are seeking an experienced and dynamic Director of Case Management (RN) to lead and coordinate a comprehensive case management program in a full-time leadership role. This position oversees critical components of the inpatient care continuum including care facilitation, utilization review, case management, and discharge planning.
Key Responsibilities:
Direct the implementation and ongoing management of the case management program at the facility level. Ensure effective care coordination and resource utilization while promoting optimal patient outcomes. Lead and supervise the case management team to drive safe and timely discharges. Partner with clinical teams and external stakeholders to reduce readmissions and manage length of stay. Monitor key performance indicators and lead quality improvement initiatives. Collaborate with medical staff, nursing, and ancillary departments to ensure comprehensive patient care. Minimum Requirements: Graduate of an accredited nursing program with an active, unencumbered RN license in Iowa or compact state. AHA Basic Life Support (BLS) certification within 30 days of hire. Minimum of 2 years' experience in case management, utilization review, or discharge planning. At least 2 years of hospital-based nursing experience. Current Iowa Mandatory Reporter certification for Child and Dependent Adult Abuse.
