Job Details

Director of Case Management:

The Director of Case Management will lead a strong and experienced team of RN's and Social Workers. The Director will oversee the discharge planning process, acts as an advocate to patients and families, provides ongoing training and development, ensure policies, procedures and regulatory compliance.

Qualifications: * Must be a Registered Nurse or Licensed Social Worker * Qualified candidates will have 3 or more years of Hospital Case Management experience including Acute Case Management, Inpatient Rehab and/or LTAC * Bachelor's prepared * Leadership experience as a Supervisor level or higher required * Certified Case Manager (CCM) preferred * BLS Certified * Active RI State License
