The Clinical Director plays a crucial role in direct supervision and training of primary care providers (PCPs) within their center, ensuring clinical excellence, quality improvement, and exceptional patient satisfaction. A portion of your time will be dedicated to providing direct patient care as a PCP, while the remainder will focus on leadership responsibilities within our dynamic clinical team. Key Responsibilities: Deliver high-quality care to patients with acute and chronic conditions, especially among older adults. Take accountability for patient outcomes, collaborating with specialists and ensuring thorough involvement in patient care. Maintain open communication with hospitalists regarding your patients' hospital stays. Conduct assessments, diagnoses, treatment management, education, and care coordination for complex health needs. Lead a dedicated care team, including care promoters, facilitators, and coordinators for in-office patient care. Engage with transitional care teams for patients unable to visit the office due to health limitations. Facilitate weekly meetings like Super Huddle and Transforming Care Meeting, supporting overall clinical leadership efforts. Assist in managing center operations, including covering for providers and actively participating in patient recruitment. Mentor and coach PCPs to enhance clinical performance, engaging with daily huddles within the center. Collaborate with Center Operations Director to improve financial performance and center membership. Knowledge, Skills, and Abilities: Proficient in Microsoft Office Suite and relevant applications. Willingness to travel locally and occasionally regionally/nationally for training and recruitment. Fluent in English, with additional language skills preferred for select centers. Ability to exercise independent judgment effectively. Education and Experience Requirements: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty (active state licensure required). At least 1 year of clinical experience in a relevant practice setting preferred, ideally with prior experience as a ChenMed PCP. Completion of Chen Medical training, including Masterful Conversations and meeting facilitation. Preferred board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty (Board Eligibility required). Current DEA registration for controlled substances.
