As a Clinical Director, you will play a pivotal role in supervising and training primary care providers (PCPs) at our center. You will be responsible for ensuring top-notch clinical quality, enhancing patient outcomes, and elevating clinician and patient satisfaction. While you will be involved in patient care as a PCP, you will also be heavily engaged in leadership responsibilities aimed at fostering a collaborative culture and seamless operational synergy. Key Responsibilities: Provide comprehensive care for older adult patients experiencing acute and chronic illnesses. Take ownership of patient care and outcomes, coordinating with specialists as needed. Engage actively with hospitalists for hospitalized patients. Manage assessment, diagnosis, treatment, education, health promotion, and care coordination for complex health needs. Lead a care team including medical assistants, care facilitators, and coordinators to deliver exceptional care. Coordinate with transitional care teams for patients unable to visit the office. Facilitate weekly Super Huddle and Transforming Care Meetings, collaborating with leadership as necessary. Contribute to the management and oversight of center operations, including patient recruitment and provider interviews. Mentor and coach PCPs to ensure outstanding clinical outcomes. Partner with the Center Operations Director to enhance financial and membership growth. Perform other duties as assigned to support the team and patient care. Required Qualifications: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty. Active medical licensure in the state of employment. Minimum of 1 year of clinical experience, preferably in a geriatric or family practice environment. Completion of ChenMed training as part of your individual development plan. Board certification or eligibility in relevant specialty. Possess a current DEA number for controlled substances. Basic Life Support (BLS) certification required within 90 days of employment.
