The Clinical Director Associate plays a critical role in supervising and training primary care providers (PCPs) within their center. This position is accountable for enhancing clinical quality, efficiency, outcomes, and satisfaction for both clinicians and patients. The director will balance their time between providing direct patient care as a PCP and fulfilling essential leadership responsibilities, such as performance enhancement and cultivating a positive clinical-operations culture.
Key Responsibilities:
Provide comprehensive care for older adult patients with acute and chronic illnesses.
Takes accountability for patient outcomes, collaborating with specialists as necessary while remaining involved in patient care.
Engage with hospitalists when patients are hospitalized, regardless of the hospitalist's affiliation with ChenMed.
Perform assessment, diagnosis, treatment, management, health promotion, and care documentation for complex health conditions.
Lead a dedicated care team, including medical assistants, care facilitators, and coordinators.
Coordinate care for patients unable to attend the office due to hospitalization, SNFs, LTC, or being homebound.
Facilitate the Super Huddle and Transforming Care Meeting on a weekly basis and support center clinical leadership as needed.
Play an active role in the center's management and assist with patient and provider recruitment.
Mentor and coach PCPs to achieve outstanding clinical outcomes, including observing daily huddles.
Collaborate with Center Operations Directors/Managers to improve financial performance and increase patient membership.
Carry out additional duties as assigned to meet organizational needs.
Skills and Qualifications:
Proficiency in Microsoft Office Suite and various company applications.
Willingness to travel locally or nationally for training and recruitment.
Fluency in English required; additional language proficiency a plus.
Strong independent judgment skills are necessary for this role.
Education and Experience:
MD or DO with specialization in Internal Medicine, Family Medicine, Geriatrics, or a related field is required.
Active MD licensing in the state of employment is mandatory.
Minimum of 1 year of clinical experience in a geriatric, adult, or family practice setting preferred.
Completion of ChenMed's training programs is a requirement.
Board certification in relevant specialties is preferred; Board eligibility is required.
Current DEA number for controlled substances and BLS certification from a recognized organization required within 90 days of employment.
