The Clinical Center Director plays a crucial role in supervising and training primary care providers (PCPs) at our facilities. This position is vital for enhancing the execution of our core model, focusing on clinical quality, efficiency, outcomes, and satisfaction for both clinicians and patients. Your responsibilities will include: Providing comprehensive care for patients dealing with acute and chronic illnesses, especially in older adults. Taking accountability for patient care and outcomes while collaborating with specialists when necessary. Engaging with hospitalists for your patients who require hospitalization, ensuring continuity of care. Managing assessment, treatment, management, and documentation for complex health needs. Leading your care team, which includes medical assistants and care coordinators, to optimize patient visits. Coordinating care for patients unable to visit the office by working with transitional care teams. Facilitating weekly Super Huddles and Transforming Care Meetings while supporting center leadership when needed. Actively participating in center management, assisting with patient recruitment, and interviewing potential new providers. Mentoring and coaching PCPs to achieve excellent clinical outcomes while participating in daily huddles. Conducting leadership rounds to strengthen team engagement. Partnering with operations leadership to improve financial performance and expand membership. Required Knowledge, Skills, and Abilities: Proficiency in Microsoft Office Suite and familiarity with various applications used in our company. Willingness to travel for local market needs and initial training. Fluent in English, both verbal and written; additional language proficiency may be required in some locations. Ability to use independent judgment effectively. Minimum Education and Experience: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Active MD licensure in your state of employment. At least one year of clinical experience in geriatric, adult, or family practice settings preferred. Completion of ChenMed training programs as part of your individual development plan. Board certification in relevant specialties is preferred; board eligibility is required. Current DEA registration for controlled substances is necessary. BLS certification from the American Heart Association (AMA) or American Red Cross to be obtained within the first 90 days.
