Job Description The Associate Director, Social Work is a Licensed Clinical Social Worker in the state of California and works with the Executive Director of Inpatient Care Management to direct and oversee the day-to-day functions of social work and discharge planning services across Cedars Sinai licensed inpatient and outpatient areas. Social work services include, but are not limited to, direct services, advocacy, counseling, service coordination, crisis intervention, and discharge planning as they relate to healthcare and discharge planning needs of the patient and family. Develops, implements and maintains a comprehensive and fiscally sound social work program and orchestrates cohesive patient care practices which address the psychosocial needs of the patients from admission through discharge and across the continuum of care. Responsibilities: Directs and oversees the day-to-day functions of social work and discharge planning services across CS licensed inpatient and outpatient areas under the scope of Inpatient Care Management Leadership in conjunction with social work leadership for Social work services which include, but are not limited to, direct services, advocacy, counseling, service coordination, crisis intervention, and discharge planning as they relate to healthcare and discharge planning needs of the patient and family. Develops, implements, and maintains a comprehensive and fiscally sound patient-centered social work and discharge planning program and orchestrates cohesive patient care practices which address the needs of the patients from admission through discharge and across the continuum of care. Develops and maintains a quality budget according to the goals and objectives of Cedars-Sinai. Develops, implement, and maintain an effective productivity measurement tool for the department. Establishes and maintains effective practices that provide for optimal quality and outcomes for patients/customers. Maintains positive physician, administrative, and staff relationships as observed from comments, informal observation of problem-solving, and feedback. Collaborates with entities outside of CS such as state agencies, medical networks, insurance companies, post-acute care providers and others in the community that may have an impact in the care of the patient. Responsible for setting strategic initiatives for social work department, to complement with CS strategic goals and completes all initiatives timely and effectively. Set goals for employee engagement and works with social work leadership on plan to achieve standards to meet goals based on employee engagement scores and feedback. Monitors and evaluates social work services through periodic review of documented psychosocial assessments performed by social work staff. Coaches and mentors staff to ensure each patient and family/caregiver interaction is approached from a perspective of cultural respect and awareness. Demonstrates principled and adept leadership that builds a highly motivated and high-performance team consisting of diverse healthcare disciplines that work well together and excel in cooperation. Designs, implements, and maintains social work orientation and on-going education of new and existing staff to ensure staff is competent to perform duties. Qualifications: Five (5) plus years of clinical experience in an acute care hospital setting, preferably in Academic Medical Center. Master degree in Social Work (MSW). Licensed Clinical Social Worker (LCSW).
